Al-Aqsa Community Clinic is committed to upholding a safe and healthy environment for both our patients and our staff. Upon your arrival, please be sure to notify the receptionists if you are experiencing any COVID-like symptoms. We also ask that all patients please wear a mask while in the facility and to maintain proper hand hygiene.

- Your Last Two Pay-Stubs
- Verification of Identity (Driver's License or Passport)

If you are scheduled to have a virtual appointment, you will receive a courtesy reminder call 2-4 days prior to your appointment to confirm. If the receptionist leaves you a message, we ask that you please return our call by 7pm on Friday, the day before your appointment. Failure to do so may result in the loss of your appointment.
